Preserving Proper Dental Alignment

Baby teeth play a more critical role in your child’s oral development than many people realize. In addition to helping with chewing and speech, they serve as “placeholders” for the permanent teeth that eventually replace them. When a baby tooth is lost too early—whether due to decay, injury, or necessary extraction—it can create unwanted gaps and allow neighboring teeth to shift or tilt into the open space. This movement can crowd out the area needed for the future permanent tooth to erupt correctly, leading to potential orthodontic issues down the road.

Why Use a Space Maintainer?

Prevents Shifting and Crowding

By keeping adjacent teeth in their proper positions, space maintainers ensure there’s enough room for the adult tooth to emerge naturally. This can reduce the need for extensive orthodontic treatments later on, saving both time and cost.

Supports Jaw and Facial Development

Maintaining the correct spacing allows for more balanced growth of the jaw and supports proper facial structure. It can also help minimize the risk of bite problems and misalignment.

Encourages Easier Oral Hygiene

A well-aligned smile makes it simpler for children to brush and floss effectively, helping prevent plaque buildup and cavities. When teeth shift, it often creates tight or awkward spaces that are harder to keep clean.

Types of Space Maintainers
There are generally two main categories of space maintainers—fixed and removable—each with its own benefits:

1. Fixed Space Maintainers

Unilateral: Band & Loop

A common option for younger children, this device uses a small metal band wrapped around a neighboring tooth, with a loop extending into the empty space to hold it open.

Unilaterla: Crown & Loop

Similar to the band and loop, but used when a crown is placed on the supporting tooth. This can be helpful if the supporting tooth has its own restorations or damage.

Bilateral: Lower Lingual Holding Arch and Nance Appliance

Specifically designed for lower teeth, a lingual arch spans the inside (tongue side) of the lower jaw from molar to molar, preventing the back teeth from shifting forward.

2. Removable Space Maintainers

Acrylic Appliances

These are custom-fitted devices similar to retainers. They can be removed for eating, brushing, or special occasions. However, consistent wear is essential to maintain the appropriate space, and younger children may find removable appliances more challenging to manage.

3. Groper Appliance

A Groper appliance is a specialized removable device primarily used to replace missing front baby teeth. By filling the gap left by prematurely lost incisors, it not only preserves space for proper eruption of permanent front teeth but also helps maintain aesthetics and supports normal speech development. The Groper appliance is custom-fitted, ensuring a comfortable feel and natural look that boosts your child’s confidence until their permanent teeth come in.

The Placement Process

Caring for a Space Maintainer

Oral Hygiene

Encourage your child to brush twice a day and floss carefully around the space maintainer to prevent plaque or food from accumulating.

Dietary Recommendations

Sticky, chewy, or hard foods can damage or dislodge the appliance. Limit items like chewing gum, caramel, or crunchy snacks. Encourage healthier alternatives that won’t place stress on the space maintainer.

Regular Checkups

Consistent dental visits help us ensure the device remains in place and to monitor the timing of the permanent tooth eruption. If your child experiences any discomfort or if the device feels loose, contact us for an evaluation.

When Are Space Maintainers Removed?

The space maintainer stays in place until X-rays and clinical examination show that the permanent tooth is ready to erupt. At that point, the appliance is simply removed, allowing the adult tooth to grow into its proper position. This process helps your child avoid preventable alignment problems and can simplify any potential orthodontic treatments they might need later on.
